Our in-office dental membership plan

Routine dental care should not depend on whether your job comes with benefits. If you do not have dental insurance, you can pay one annual fee to our office and get your cleanings, exams, x-rays and fluoride for the year, with member pricing on anything else you need.

It is a membership plan offered directly by our practice, and it is worth being clear about that before anything else. There is no insurance company, no third party administrator, and no claims are filed for members. The agreement is between you and our office, the care is delivered by our team here in Turlock, and the price you see is the price you pay.

Choose the plan that fits

Each plan runs for 12 months from the day you enroll and covers one person.

  • Perio Focus Adult Plan

    $950 per year

    Adults whose periodontal condition calls for maintenance cleanings on a three-times-a-year schedule. Which plan you qualify for is a clinical decision made from your periodontal charting, not a choice at the front desk.

    $550 per year

    Adults with healthy gums who are on a routine twice-a-year cleaning schedule.

    Children and teens. Dr. Kaci sees patients from twelve months old, so the whole family can be on one schedule.

Which adult plan you qualify for is a clinical decision based on your periodontal charting, not a preference. We will tell you which one applies after your exam, before you pay for anything.

Member pricing on everything else

Treatment beyond your included visits is charged separately, and members pay a reduced fee for it at our Turlock office. Call us at (209) 667-0115 for our current member fee schedule. You will always get a written estimate before any treatment begins.

What the plan does not include

Being straight about this up front matters more than a longer list of benefits. Your plan covers the visits named above. Everything below is charged separately.

  • Any treatment beyond the services listed in your plan, including fillings, crowns, root canals, extractions, night guards and whitening
  • Deep cleanings, also called scaling and root planing, and the periodontal therapy that follows them
  • Orthodontics, including Invisalign
  • Dental implant placement, and any other procedure we refer out to a specialist
  • Care from any other office, including specialists we refer you to
  • Laboratory fees, sedation and prescription medications
  • Missed appointment fees

Plan terms

These are the terms you agree to when you enroll. We would rather you read them now than be surprised later.

  1. This is not dental insurance. It is a membership plan offered directly by Dr. Kaci Kuykendall, DDS.
  2. The plan is solely between you and our practice. There is no insurance company, no third party administrator, and no claims are filed for members under this plan.
  3. This plan is not a qualified health plan under the Affordable Care Act and it does not provide minimum essential coverage.
  4. Membership is available to patients who do not have dental benefits of any kind. It cannot be combined with dental insurance, with a government dental program, or with any other discount or offer.
  5. Benefits apply only to care provided at our Turlock office at 1600 Colorado Ave, Building 3. Care from another office, including a specialist we refer you to, is not included and does not receive member pricing.
  6. The membership fee is due in full when you enroll and covers a 12 month term beginning on your effective date.
  7. Included services must be used within your 12 month term. Unused services do not roll over, cannot be redeemed for cash, and have no value after your term ends.
  8. The membership fee is non-refundable and non-transferable. One membership covers one named person and cannot be substituted to another family member.
  9. Your membership has to be active and paid in full for you to receive included services or member pricing.
  10. Services outside your plan are charged at our regular fees. We will give you a written estimate before any treatment begins.
  11. We may change plan pricing and terms for future plan years. Any change takes effect at renewal, and a membership already in force is honored through the end of its term.

Common questions

Is this dental insurance?
No. This is a membership plan offered directly by our practice. There is no insurance company involved, no third party administrator, and no claims are filed. You pay us once for the year and receive the services listed in your plan here at our Turlock office.
Can I use this with my dental insurance?
No. The membership plan is for patients who do not have dental benefits. If you do have a dental plan, call us at (209) 667-0115 with your plan name and member ID and we will look up what your benefits cover here before you decide anything.
How do I know which plan I qualify for?
The difference between the Perio Focus plan and the Standard plan is clinical, not preference. Patients whose periodontal charting shows they need maintenance cleanings three times a year belong on the Perio Focus plan. If you are not sure which applies to you, we will tell you after your exam, before you pay for anything.
What happens if I do not use all my cleanings?
Included services have to be used inside your 12 month term. They do not roll over into the next year and they have no cash value. We will help you get scheduled so nothing goes unused.
Does the plan cover fillings, crowns or root canals?
No. Those are additional services and are charged separately at our regular fees, with member pricing applied. Call us for the current member fee schedule. We give you a written estimate before any treatment starts.
Is the membership fee refundable?
No. The fee is due in full at enrollment and is non-refundable and non-transferable. It covers one named person and cannot be moved to another family member.
How do I sign up?
Call us at (209) 667-0115 and ask for our financial coordinator. She will confirm which plan fits, walk you through the agreement, and set up your billing. You can also request an appointment and we will go over it at your visit.

Have dental insurance instead?

The membership plan is for patients without benefits, so if you have a dental plan this is not the right fit. Call us with your plan name and member ID and we will look up what your coverage does here before you schedule.
